Averett University's Student-Athlete Medical Forms
The Averett Sports Medicine Department requires all student-athletes to fill out the proper medical forms in order to participate in intercollegiate athletics. These forms allow the student-athletes to complete the forms electronically to send them to Averett's Sports Medicine staff.
